So all my icache flushing troubles appear to have been bugs in the
platform code.  With these corrected, it now works.

(At least until I enable HIGHMEM, which seems to cause *other*
platform-specific bugs to bite.  Serves us right for picking a 6 month
old all-new CPU design).

Anyway, it's a little frentic here now, but after things settle down
I'll put together a clean patch for mips.  In the meantime if anyone
really wants vmadump on mips they can pester me for an ugly hack of a
diff...
