#38 Allow meta modules entities

When the same sequence of modules is used for various
manuals it should be usefull to be able to gather them
in an entity file.
For that to work it is necessary to allow entities
expansion before module elements are placed by module
entities in master.top.xml
This implies getting rid of XML::Twig and use
XML::LibXML instead.
