#32 Blacklist based on combination of IP address and EndpointURL

Should be blacklisting based on both IP address and Endpoint URL instead of just Endpoint URL.

The basic Java HTTP connection classes do not allow access to the actual IP address at the end of the connection, so this feature will require a change to a more allowing HTTP connection library such as Apache HttpClient.
