At Beth Israel reports to referring physicians include attachments to show slices where pathologies are indicated, many times with arrows. Likewise it is common to include an animated GIF file to show the rotating MIP (see demo).
The data is stored in parallel on the hospital PACS which supports the use of significant images. All 3 of these can be accomplished by the single press of a button.
The Report looks like

For the original BI database, the button which supports all 3 is the middle button ** Jpeg+Significant Image + MIP**. The top button ** Jpeg -> DB ** saves the current slice to the database. Later on a separate program for generating the reports for the referring physician will query the database and attach any files which have been saved.
This program has been extended for use with Orthanc. Orthanc stores DICOM files, not GIF and JPEG. Instead of -> DB, the gif and jpeg files are stored in the location to the left of
. Pressing on the icon allows setting the location. The files here can then be attached to your reports. The button Jpeg+Significant Image + MIP puts the jpeg and gif in the specified directory, while storing a copy of the significant image in either Orthanc or the hospital PACS.
Note the ** current study ** box which says ** Click on an image and then back here**. This message may initially appear if you have not yet chosen any image. By definition a report is connected to an image set and clicking on the image defines the parameters of the report.
The most common report is for a PET CT study. Only for it is there a MIP rotating image and the middle button makes sense. Upon clicking on the Pet-Ct Viewer and then back to the report you will see

The patient data have been anonymized. You can see that the frame size of the jpeg will be 704*409. If the size of the email attachment is important, it may make sense to shrink the window size before saving the report. The information shows that you are currently displaying PET slice 87 of 131 slices, and CT slice 142 of 488 slices.
The other use for this tab is for making teaching studies (BI database only)

In Reading studies the last column is for Teaching studies. Teaching studies at displayed for going over interesting studies for the residents. The database may be queried for teaching studies. The second study in Reading studies has a label indicating the type of teaching study.
On this Report form, the first text box defines the label which will appear in the teaching column. The large text area box is for giving all the details which will be stored together with the particular series which is chosen. The ** Update ** button will become active as soon as a change is made in one of the above text boxes. The ** Update ** will save the changes to the database.
Note that the Pet-Ct viewer as such is not stored on the database. Only the source data is stored. Thus all teaching studies refer to source data even if the data of interest is Pet-Ct.
Finally note the ** Cine (or MIP) -> DB ** button. For Pet-Ct this button will be "pressed" automatically as part of the 3 button. For a non Pet-Ct like a cardiac study it may be interesting to include in the report an animated GIF of the beating heart. This button creates a Cine of the source data.
