#4 File Parameters are not shown on exporting to CSV

The File Parameters dialog is not shown to the user if the export to XLS was done previously.

Ok, you are 'half' right :-).

The problem is that the dialog box is not big enough to show the 'Separator' and the 'Quote Char'.
It is only big enough to show the 'Save Columns Name'.
But of course you can pull the right lower corner to make the dialog bigger.
That was something that I currently noticed.

... Strange, the dialog box is (theorically !!) adapted to the context. But there is a problem in the form position (not centered), corrected in the next release. I attach a new version with fixes, so you may test if the fixes solve you problem (or not !)
