#98 Support for user configurable block "starters"

I'm trying to reformat some C++ code. They use the BOOST_FOREACH macro. This macro is like the foreach of C# and so it should start an indented block, and if using some options, it should cause a { and } to be added to the code if missing. Clearly astyle can't know all these macros, so it would be beautiful if you could add them as an option (so something like --blockstart BOOST_FOREACH,MY_FOREACH or something similar)
