I noticed the addition of the ManagerCommunicationException to the originate
operations on AsteriskServer.  

I am guessing that in theory it would now be possible for you to throw an
Exception with the origination failure cause contained in the exception.  In
that case you'd never have to return AsteriskChannel = null.

Is that the plan?
