As far as the AppendToGatewayUrl thing goes, you can solve the issue by subclassing NetConnection and adding the following methods to that subclass: AppendToGatewayUrl, AddHeader, ReplaceGatewayUrl. As far as the user error, I don't know... Can you capture the incoming AMF be adding the line $gateway->logIncomingMessages('path/to/incoming/messages/'); in gateway.php and send me a copy?
