This may be a bug in some old ALSA version (before 1.0.8).

The startup script should call the hotplug script when booting (this
is called coldplug).  All recent distributions do this automatically.

hw:X,0 -> /dev/midi0X
hw:X,1 -> /dev/amidi0X
