[AD] Adding html text substitution to makedoc

The following patch adds a new command to makedoc. With this command
the HTML output can replace specific strings of text by another
text. This can be used effectively to make readme.txt a hyperlink,
which should improve usability. It is generic, so it could be used
for other things as well.
