The biggest improvement from v0.4.5 is visual.  It simply looks better
and has better navigation and it's easier to customise navigation in
the DSSSL file.  Also added a preliminary print-stylesheet in case you
want to experiment with printed documentation.  Several other changes,
please check Changelog.
