[agilewiki-wiki] Response DTD Elements

Some good progress. I've now defined all the elements (but not yet the attributes) in the response.
   
  I found two duplicate element names, Time and Pathname, and changed the code so that we now have Time, HTime, Pathname and HPathname. Having a real DTD is a valuable thing, so it is worth changing the code so that we can have one.
   
  I also found 3 unused elements in the client code: Table, List and KeyedList. In this case, I have not changed the code.
