[AgileWiki] Comments revisited

Two changes made to comments now:

1. The comment form includes ta display of the document you are 
commenting on. Makes it a lot easier to cut & paste several items, as 
well as making it easier what you are commenting on. :-)

2. Once you create a comment, you are returned  back to the document you 
made the comment on.
