#17 Add checkbox to CDROM dialog

When a CD reference is enabled in the Adept
repositories but the CD happens to be unavailable,
often a dialog requesting the unavailable CD appears
and reappears over and over again. This slows
progress installing software to a crawl. It is often
faster to restart an install session with an altered
repositories list.

This could all be made quicker and arguably more
effective if the dialog would include a checkbox "Do
not show this dialog again", which, if checked, would
cause the configure/download/install process to
bypass the particular CD repository. For convenience
and to expedite the install, the checkbox could be
checked by default.

Please implement this change at your earliest
convenience.
