Using v5.4.3 ANSI (because I couldn't get Unicode Function List 2.0 to work on ASP files, but that's another issue!):
Went into Settings/Preferences/Global, and selected "Big Icons" -- the toolbar became blank. "Small icons" are all there, as are "small standard icons".
Any ideas from anyone?

Works for me with both ANSI and Unicode, no icon themes around. What is your OS, Does restarting Notepad++ help?
