For the menu items which perform the same command as some toolbar button, the corresponding icon (or the standard 16x16) can be added easily in the code.
Using icons on menus could be toggled on the fly on the Prefs dialog, and kept in config.xml (default=No icons, for compatibility).
For people who would like to add more icons still, the technique used in toolbarIcons.xml could be used. Or even the file itself, though it would have to be renamed.

Totally agree with you!
Eventually this could be optional, not the default.
For now you can vote for http://sourceforge.net/apps/ideatorrent/notepad-plus/ideatorrent/idea/73/. This will help pushing it closer to the top of the agenda, though it is somewhat crowded.
Is this functionality to extend to plugin menu commands?
