even after selecting all the options several times, the icon bar is missing?
I've even compiled the thing myself.. what happened?

Perhaps something in your settings? Try making a backup (or renaming them) and create new ones from the .model.xml files.
i've upgraded to v5.0 and there is no icon bar. how do you turn it on? or off and then back on again?
The settings for hiding the Tool and Menu bars should be under Settings > Preferences > Global (tab). Have you tried changing those?
