This scripts allows for up to 10 questions, of administrators liking. Dynamic configuration like requesting name, phone, mail, address etc etc. It allows for comments when voting. It also works without interfering with your address line. A detailed display of satisfaction, along with the option to print out comments and user details list/print friendly way.
