Updated unit test for MediaPlayers new default width/height. FM-300.

Fixed a problem where the SMIL plugin was ignoring any metadata on the resource when it created a new resource for rtmp dynamic streaming. This was preventing media from playing when both the MAST plugin and the SMIL plugin were loaded.

Another minor cosmetic change to AkamaiPluginSample

Minor cosmetic improvements to the AkamaiPluginSample app.

Add metadata to the resource of elements created by LoadFromDocumentElement subclasses indicating the derived resource. This allow recipients of MediaFactoryEvent.MEDIA_ELEMENT_CREATE to be able to determine where a created element derives from. Updated F4M code and SMIL code, plus unit tests. Also added MediaType to resources generated by SMILMediaGenerator, in case they lack file extensions.

1) SMILPlugin - SMILMediaGenerator class modified to support both live and vod smil files. SMILLoader modified to retain original resource metadata.
2) AkamaiBasicStreamingPlugin refactored. This includes improved support for all types of streams while the plugin is loaded, added support for plugin metadata that can override built in defaults for a) live stream time out; b) whether or not the plugin should try to reconnect to a live stream if both primary and secondary encoders fail; c) the interval at which the retry should occur. Added support for resource metadata for specifying connect-level and stream-level auth tokens so that these tokens do not need to be part of the URL (although this is acceptable also).
3) AkamaiPluginSample app updated to support auth tokens which are not part of the URL (these are passed to the Akamai plugin via resource metadata).

Minor cleanup, fix typos.

Better fix for serial seek trait event order bug. Also fixed bug in MediaPlayer where currentTimeChange event could be dispatched on trait addition, even if currentTimeUpdateInterval was zero. Added unit tests for SerialElement holding DurationElements, since that case exposed quite a few edge case bugs.
