Initial check in for logging framework

Make resolver a generator constructor param, as it's unlikely to change.

Revert previous change, the resolver actually is specific to the generator class, and not strictly germane to the model classes.

Move resolver classes to model package, since they're primarily focused on working with the model objects (and not integrating with the framework).

Modify resolver to return MediaElement, not index. Update some comments. Remove unused TextElement.

Prototype: Add support for retrieving VAST wrapper elements. Introduce HTTPLoader for retrieving data from URLs. Minor changes to VAST object model and parser (just the minimum to get wrapper parsing to work), probably needs a formal pass.

Dynamic streaming unit tests now pass.

Adding a missing file.

Improvements to the VAST document object model.

disabling NetStreamSwitchable until the Unit tests can be fixed.
