I have the same problem. I changed SMIL file for our mp4 files and every time I have error: "The HTTP loading operation failed". I even downloaded their SMIL file and put it on my server with same problem.
Inside Model.as there are lines :

Hi everybody,
We changed "live" application on our FMS server to other name (security issues) and if
we use OVP for Live streaming counter doesn't count. If we change to "live" everything is OK.
Is any class in OVP predefined to "live" application only? Can I changed to our live application
name?

I tried to use OVP Dynamic Streaming for Live Dynamic Streaming and it didn't work.
I changed I don't know on how many ways SMIL file without success. Is there any additionally
way how to change OVP for Live Dynamic Stream?
