Happens in both iText 2.0.6 and 2.0.7
Happens both when compiling in JDK 1.6 and 1.4, on both Debian and Win32
Using bcprov-jdk14-138.jar/bcprov-jdk16-138.jar
The program at http://vladr.dyndns.org/pub/pdfsign.zip stamps an existing PDF with a digital certificate, applies an image on every page, and saves to a new PDF.
Once every 3-6 runs or so I get...

Probably has to do with Vista's improved security and/or support of short file names (I don't remember how this worked.) But VDMSPATH should be set IIRC, and should possibly be a valid 8.3 (short) file name version i.e. C:\PROGRA~1\VDMSOUND, not C:\Program Files\VDMSOUND (not sure about that last one tho'.)

this is more likely caused by an external application. does it also happen with sound disabled in the game? if so then the external application in question is very likely sound-related.

Check that the VDMSPath environment variable is properly set, that's where the DLL is normally searched for.
