Yes, I'm using auto-reload and pdfsync. (Indeed, quitting Skim and restarting it seems to have fixed the problem for now.)
Also, when the proxy icons stop working, they stop working for all files, not just the ones being auto-reloaded.

For some reason dragging the proxy icon doesn't seem to work. I'm using Version (0.9.3) (v15).
