some answers to your questions:
1) If what you're saying is correct, this seems like a real problem. What I can image that such a mail gets processed, but is stuck somewhere in the process... On my side, I sometimes see spamatoxy processing unread mails and suddenly stops. I then click thunderbirds "get mail" button and it continues...

actually a quite nice idea you are presenting here. but i think, spamato(xy) is not (yet?) the right tool to do server-side filtering - and that's what i see here.
the main reason - in my opinion - is that spamato(xy) is very strong in user specific filtering. So in it's current existance, it makes much more sense to have it installed on each users pc.
Such a "server-side".

the next spamatoxy release will handle "services" in a more advanced an general way. no possibility to have multiple services on the same port...

next spamatoxy release will be different regarding service configurations.
there are no "services" that have to be configured, just 4 standard services for the four types of connections (pop, pops, imap, imaps).
additionally, the remote mail server will become a part of the "user" field in the email-client...

as expected the TOP command was treated the wrong way. actually, I think TOP never worked.
it's fixed in CVS and will be in the next release. if you need it earlier, drop me a mail to andi (at) spamato (dot) net.

seems to be the same problem as in bug 1791981. it's not a problem of apple mail, there seems to be a misbehaviour in the TOP command handling...
