Well, the rear.log says that you successfully created an ISO image ... ?
Unfortunately I don't have a RHEL for testing available at the moment. Maybe you try to install a proper vol_id, e.g. the one I put on https://sourceforge.net/projects/rear/files/vol_id/SLES9SP3 (because SLES9 had the same problem).
I actually would recommend to install the vol_id RPM (or rebuild it from the src.rpm...

and as usual we need to see the log file, also from the rescue system. You can use ssh to/from the rescue system, for ssh to the rescue system you must use ssh keys.

Please try this:
cd /usr/share/rear/prep/RedHatEnterpriseAS
mkdir 4
cd 4
ln -s ../ia64/05_vol_id_function.sh
and try again. I will try to find out why our tests with RHEL4 where successful. If this works for you then I will put a "correct" solution into ReaR.

can you please play around with udev_volume_id on your system and try to get it to recognize your LV? That is the root cause of the problem and without access to your system I can only guess here.

so what kind of filesystem is on that LV? We use the vol_id or udev_volume_id from udev to read file system details.
I remember doing something in this area, please check with 1.7.21 and submit the log file as an attachment and the configuration files you touched. What is strange that in the log file I don't see
Using 'udev_volume_id' for vol_id
which I would expect on a RHEL4. Take a look...
