I just tried fop 0.95beta and can confirm Mauritz's note. The problem does occur with 0.94, but not with 0.95beta. So, I am closing this bug with the note "Works for Me," which I take as equivalent to "No Change." For anyone using FOP 0.94 or earlier, removing the line as suggested in the original report is a workaround.

Re-opened at Bob's suggestion, with the recommendation that the fop1.extensions setting should turn off the extra page automatically.

Turns out this happens because of a bug in FOP, and doesn't happen in XEP.

The titlepage.templates.xml file inserts an extra page between the title page and the Table of Contents. That is, the standard stylesheets produce recto and verso title pages, then produce an additional page. This forces the Table of Contents to a verso, rather than a recto page.
