I think I found the problem...
The display glitch happens *only* if I'm using a customized theme.
If I go back to WinXP's standard theme(s) ( Display Properties -> Appearance -> Windows and Buttons -> choose Windows XP or Windows Classic ), the glitch won't happen.
So, it's either (1) fwb can't handle custom themes, or (2) my theme is corrupt.
(and I can confirm that running UltraMon on...

@vkurland: It's from "Ultramon". The double-arrow moves the window to another monitor (I have dual monitor), the other icon maximizes a window across both monitors.
I tried disabling Ultramon; no effect. Non-maximized MDI windows still get cropped.

I've just installed Ubuntu 9.04, with the LAMP package and PostgreSQL package.
Configured PostgreSQL afterwards, install phppgadmin and all is well.
Then I install bandwidthd-postgre using aptitude, ln the /var/lib/bandwidthd/htdocs directory to /var/www/bandwidthd
Restart Ubuntu, all [OK].
But no index.php ever pops up in /var/lib/bandwidthd/htdocs
What gives?.
