I can confirm that this template problem is an issue using the .oxt installer now provided, at least in OOo 3. Initially, OOoLilypond tries to configure itself using an obviously temporary install folder as the location of the templates. In the case of my system, it didn't actually *create* this folder or any of the template files on install.
