We are working on this item and need more information to reproduce. Would you pls update this ticket with a sample DITA topic + image that is producing the bad result? Also include a screenshot of the resulting *wikitext* markup if possible.

I assume your issue was not resolved by the Theme Builder forum suggestion?
I use the Theme Builder GUI to customize layouts with left navs etc. What approach are you using? A step by step and maybe some screen shots would help me course-correct you if needed.

I suggest posting your questions regarding pagetree2 and its customization options to the Theme Builder community forums. Google "adaptavist theme builder" and you'll find a path to answers.
I will say that from your description, it looks like you're mixing two different implementation approaches (Confluence TreeNavigation and Adaptavist pagetree2) in order to create a logical tree...

Just updating this thread to note that Adaptavist has now open-source their Theme Builder plugin, thus enabling you to e.g. build dynamic navigation trees based on your DITA maps with fully open-source components. Th DITA2Wiki Project highly recommends this plugin as part of any DITA2Wiki solution.

Just to update this thread with some good news: Theme Builder is now in the open source. The DITA2Wiki Project recommends using this plugin, together with the open-source "pagetree2" macro, to dynamically build logical tree views based on DITA maps.

I was able to repro this on my local instance with your src. The culprit appears to be the Topic (base) DTD:
* When I use the Topic (base) DTD, I get a dupe title.
* When I apply another DTD, say, Concept, I don't get a dupe title.
