This usually happens when there is a funny character in a bookmark title from Firefox since the bookmark titles become the filenames in Internet Explorer for favorites.

The information regarding the Qt 2.x requirement is outdated. BookmarkBridge 0.76 was written and compiled using Qt 4.x. It's not easy compiling BookmarkBridge on Windows, as it has several dependencies such as libxml2.

There's a bug that causes bookmarks to get incorrectly flagged as readonly when manually added. Edit the bridgeconfig file shown above and remove the <readonly/> tag. Be sure BookmarkBridge isn't running at the time. Delete the bookmarks.xbel file and restart BMB. Try the merge again.

The current version of BookmarkBridge wasn't designed to syncronize bookmarks between separate PCs. If I can find the time, a future version will support this.

Follow the FAQ on the web site at http://bookmarkbridge.sourceforge.net/faq.html for manually adding a browser. You'll need to set both your Firefox HTML bookmark files as sources and select your fresh Firefox installation as a destination.
