You can use the module parameter 'if_name=wlan%d' to change the default behavior to use wlan%d instead of eth%d.
modprobe ipw2100 if_name=wlan%d
or in your modules.conf you can add it as an option
options ipw2100 if_name=wlan%d.

0.46 includes a few changes that could fix this; can you let
us know if you still see this problem with 0.46 (or newer)?.

0.46 might help _some_ with your start up issues (it should
solve the 'May 24 00:49:01 localhost kernel: ipw2100: attempt to
use fw ordinals before they have been loaded.' messages).
Those messages could relate to future problems the driver
would have.
For the C3 issue, the processor patch just keeps the
processor from moving into the C3 low power state...

0.46 includes changes to check the state of the RF switch
and keeps the radio from attempting to initialize if the
switch is in the 'radio disable' state.

I looked through the code some more; I found an opportunity
for the timer to not be cleaned up during init with 0.46_3
/if/ the hardware fails to initialize for some reason.
Attached is a patch; let me know if it helps.

What do you mean by 'cut off'? The network just stops
working (no data sent/received) or do you lose association?
What are you running when you notice the connection stalling?
In additon to the above information, could you attach the
output of dmesg after you experience one of these stalls?.
