My protocols are AIM/ICQ and Jabber and the smiley theme I use is Trillian-Smileys from kj (TrillianSmileys + Extension v0.1). The smileys are sometimes animated.

It also happens often for me under Windows XP with SP2. I have the feeling that it happens more frequently when Gaim diplays mixes of lines with smileys and without. It happens more often when a line containing no smileys is displayed after a line that contains smileys.
