Actually appearing before the TOC isn't the issue. The dedication and epigraph (if at all) appear before the TOC, and are listed. But those have content that is part of the authorial work, and more important, corresponding titles on their (first) pages.

The manual (http://ogs.ucsd.edu/AcademicAffairs/Documents/Dissertations_Theses_Formatting_Manual.pdf) does not require the signature page to appear in the table of contents. It does appear in the sample TOC, but samples are just samples unless a rule is stated definitively elsewhere.
I agree it's unconventional to list in the TOC items that appear before it. I recommend thesis authors push...

Thanks! I fixed this in a less elegant way (in revision 1.2) just before seeing this patch. Revision 1.3 includes a better fix inspired by this suggestion.
