Thanks for the tip; This is definitely going in our new Spamassassin ruleset that is due to be rolled out soon. (The current set obviously isn't doing a good enough job.)

I'm sorry some messages have gotten through; We recognize the current SpamAssassin ruleset isn't ideal (and parts are a bit dated). Rest assured, we're testing a new set for deployment Real Soon Now.

We've previously submitted this as a bug notification to the maintainers of exim. There's not much more we can do in the meantime.

In addition to the situation you describe with the duplicate file names, we occasionally have a transient problem on the SourceForge side that causes the 553 error (as in the initial ticket). We try to stay on top of things and catch it when it happens, but sometimes it can take up to 24 hours to remedy.
