Is your keyboard acting up?
Remember ... when you type in a password in Terminal, it doesn't look like anything is happening, the cursor stays put, but the password is being accepted ... just type it accurately and hit Return. Isn't that right?
Don't have anything plugged into any extra USB ports on the KB either ... that seemed to challenge my Aluminum Pro KB awhile ago ... messed KB...

Which G4 is it? What is the RAM size? Which video card? What PCI cards installed? Have a good PRAM battery?
Are you using the CUDA button pressed and opt-cmd-P-R at new Startup?
I believe you have to have AppleJack installed (with memtest option) from a good OS X partition ...
Could you possibly just install 10.4 ... not go "up" from 10.1. Is that what you did? 10.1 up to...

Don't know much about this, but you could try manually dragging this file to the Trash, and on Restart it should be regenerated. Then you could empty the Trash. You would probably also need to relist some of your Startup Items? of is that part of the regeneration?
Don't think applejack is omnipotent ... its great, but this might be one place it doesn't get to ...

Previously, I "applejack > memtest Certified" 3 fresh sticks of 512MB PC133 RAM by rotating each to each of my 3 memory slots in my Quicksilver 2002 Dual 1GHz, and ran the 3 cycles on each 3 stick configuration.
If I have 2 known good 512MB RAM sticks in Slots 1 and 3, and place a test 512MB stick in Slot 2, am I reasonably assured if it passes the 3 rounds offered by applejack's...
