For #1, here is my reasoning: I'm not sure if we want to put all the possible accounting dimensions into report parameters. What about the rest? Org Trx, Locator From, Locator To, SubAccount, User List 1/2 ?

[1] I'm wondering how important the combination is. As far as I test it, it works fine even the account combination is not created. I think we need to add reference mechanical to achieve that. Not done yet.
