A feature that would be cool, though not that important, is having a cursor(for example looking like a little pen or just a line) that would would lean in the same angle and direction as your tablet pen, like the one in Corel Painter 10+.

Some tablets registers the tilt angle and pointing direction of the pen and I think it would look cool if when the more you lean the pen, the wider the lines would get. It should also be (more blury and) more transparent longer away from the "center"(cursor).

Although not extremely important - adding a texture on the pencil tool would make it look a little more like a real pencil.

A tablet or mouse can only register a certain amount of positions at a given time, and therefore resulting in "stiff" lines when making faster strokes.
So some kind of of "smoothening" function would be nice.

At the end of every drawn line with the pencil tool(in bitmap mode) there is a transparent dot.
The bug was introduced when the tablet pressure transparent feature was updated to have a smoother gradient.
(Existing in development versions after 0.4.4b)
