editing online forms

problem with workflow

i have a workflow to use for pediatric surgery. I am using OIO live cd. My problem is to write a qusestion in one form and then fom that make it pick one of two form to fillout. i chose fillout on the workflow. but how can i make sure it works. i already created a custom response in the question answer to enact the worklfow and in the workflow it is supposed to fillout a form. but i can't...
