NullPtrException when Source has no systemId - ID: 761891

When a stylesheet module is loaded using a
user-supplied URIResolver, and the returned Source
object does not have a systemId set (ie. setSystemId()
has not been called), Saxon crashes with a
NullPointerException at line 99 in module
XSLGeneralIncorporate.
This problem was reported on Saxon 7.6 but it has been
in the code since 7.3 or earlier.
Source code fixed, not tested.
