Macron heights off for characters with long ascenders - ID: 2881176

I am having trouble getting the correct rendering for TeX sequences like
\=l. The macron appears midway through the 'l'. This occurs even with
precomposed characters, such as U+1E38 CAPITAL L MACRON UNDERDOT.
