Patch: SSPIUsernameFormat - ID: 2840094

This patch removes the directives SSPIOmitDomain and SSPIUsernameCase.
Instead of the two directives only SSPIUsernameFormat exists.
SSPIUsernameFormat: controls the username format given in REMOTE_USER. %d
and %u and synonyms for lowercase domain respective username, %D and %U are
the uppercase variants. %% is used for the sign '%'
Patch contains compiled .so against Apache 2.0.58.
