6 (Mac OS X) Crash when <convert>mt32 - ID: 2822446

On OS X, when the MIDI device type is set to MT32 in exult.cfg, Exult will
crash at some point between starting Exult and actually starting a game. If
you change it via the in game menu, then Exult will crash on next startup.

When CoreAudio is selected as MIDI device, MT32 should not be a selection
in the in game menu.

This affects all Midi drivers that have the option to convert/mt32, and
also the MT32Emu driver.

Uncommenting #543 in XMidiFile.cpp 'events[i]->decerementCounter();'
prevents the crash but that's not a good solution, I guess.
