Flash support for 29LV081B, 29LV400B - ID: 2821685

This patch supports AMD 29LV081B, 29LV400B and their compatibles.
It will fix bug #2821646 #2121629.
And it includes #2168751 enhancement (Erase all support). If you reject
#2168751, remove amd_29xx040_erase_all from the initializer of
amd_29xx0400_flash_driver.
