SMA_AS: Separate change of account number and IM - ID: 2219521

The same sequence with "AcctInfo" holds account number change fields
("ClientAcctNumChg" and "OldClientAcctNumChg") as well as investment
manager change fields ("IMChgInd" and "TerminIM"). Perhaps we should
consider separating these for clarity.
