5 Database form dropdown menu is broken - ID: 1909163

Details:

For the love of god will someone please fix the database drop down menus,
they have been broken since several releases. Why hasn't it been fixed ?

The initial database list should be in a drop down menu form but it only
shows up as bullets.

I know its silly but it should be fixed
