5 mod_authn_pam: make PAM service name configurable - ID: 1882583

The attached patch makes the PAM service name used
configurable, instead of hard-coding it as "http".
This allows to re-use an existing service, but can
go as far as creating per-directory PAM configurations
using different PAM methods.
