6 improve surt docs; esp 'surts-source-file' syntax - ID: 1436290

The requirement that SURT prefixes in a
'surts-source-file' be preceded by a '+' is
non-intuitive (and in fact a new requirement starting
in 1.6). Needs better doc in manual and web UI.

Doc/text-only change: Implementation of [ 1436290 ] improve
surt docs; esp 'surts-source-file' syntax
* user_manual.xml, SurtPrefixedDecideRule.java,
SurtPrefixFilter.java, SurtPrefixScope.java
explain that 'surts-source-file' can include regular
URLs and '+'-preceded SURT prefixes, in manual and help text
popups.
