I recently fixed this after beta was released. I double checked the given test case and trunk currently does not exhibit this behavior.
This bug is fixed.

In the "Manage MIDI Devices" window, the default MIDI output device is labeled "MIDI input system device".

** Bug Description **
In the "Manage MIDI Banks and Programs" window, switching between banks can cause patch names to mysteriously disappear.
** Reproducing the Bug **
1) Open "Manage MIDI Devices"
2) Click on "Banks" (make sure you have the default device selected, which should have GM patch names loaded by default)
3) Select the "General MIDI" bank. You will see instrument names...

It is not possible to edit the name of a bank in the "Manage MIDI Banks and Programs" window.
** Reproducing the Bug **
1) Open "Manage MIDI Devices"
2) Click on "Banks"
3) Double-click/right-click/etc/ on the bank names--there is no way to change it.

Original bug should be fixed in SVN rev 11569. Not sure about the other bug mentioned in the comment, yet. Please do test.
