Did you just install the 1.2 Murmur over the top of your 1.1.8 Murmur? That might be causing problems with the ACL - Superuser by default should already be registered, and should have the ability to do everything unless something's broken (most likely a database problem I would guess).
In fact, I believe by default users will have the ability to register themselves on a fresh server - what happens if you log in unregistered and right-click yourself?
I would definitely delete the .sqlite file and start again if it has been there since 1.1.x.
Finally, the "channel password" is not prompted for because one of the chief design goals of Mumble is to institute community trust... to you or I it might seem like the devs are just making things difficult, however if you play something like Eve where people are pretty constantly trying to find any advantage they can, then you can see why people take extra precautions.
That's not to say it's not coming... slicer's said it's something they'd like to do, they just want to do it in such a way to ensure it's not going to be able to be used for leverage. At the moment, have your users enter their access tokens in the box in the server menu once when they first connect to your server, and they'll maintain all their permissions pretty well permanently.
