Mantis is an easily deployable, web based bugtracker to aid product bug tracking. It requires PHP, MySQL and a web server. It is simpler than Bugzilla and easily editable. Check out the online Demo.

Mantis is a very full featured and mature bug tracker. If you are looking for a solution that is an integrated Configuration Management system, take a look at the Trac project.

Not the greatest UI layout, but simple, effective and fully-functional! It did everything we wanted from a bug-tracking system - and even used it to track User Stories! Love it!
